Mobilisation Complex Care Coordinator - Trusted support that sets great care in motion

Wokingham Head Office | Full-time, Permanent | 40 hours per week, Mon-Fri

£35-40k per annum DOE.

We are looking for a Mobilisation Coordinator to join our Mobilisation Manager and lead the safe, smooth, and timely setup of new care packages for our clients with spinal injuries, brain injuries, and complex conditions. You'll act as the anchor during this vital transition – bringing people, processes, and systems together to ensure that care starts not just on time, but in the right way. You’ll be the trusted support to our case managers and families that sets great care in motion.  You’ll need precision and compassion to ensure that the personalised needs are met.

What you’ll be doing:

  • Coordinate and deliver the full mobilisation journey – from referral to go-live
  • Build strong, compassionate relationships with clients, families, and Case Managers
  • Work closely with internal teams (Recruitment, Compliance, Clinical, Training, Ops, HR)
  • Create robust rotas in OneTouch, ensuring care teams are scheduled and trained
  • Spot risks early (dropouts, delays, data gaps, agency reliance) and take decisive action
  • Ensure our clients feel seen, supported, and safe from day one
  • Have outstanding people management and engagement skills because behind every great care team is a calm, capable coordinator
